Wow that jumps right out at you. I would play around a bit with noise reduction and see what you get. I think today's noise filters do much more to preserve detail. You may also try creating a duplicate layer, apply NR and then mask off the areas you do not wish it applied.

Its similar to what I have been doing with motion blur in PS. The trick is more than just the masking...its in the blending options as well to get it to look seamless.
